By repeatedly asking the question “Why” (five is a good rule of thumb), you can peel away the layers of symptoms which can lead to the root cause of a problem. Very often the reason for a problem will lead you to another question.
An example of this was a motor that had come in with a vibration issue. After a careful evaluation of the bearings electrical fluting was established as a primary cause of failure. But there was evidence of other issues as well. It was decided to do a complete engineering workup to get to the bottom or the final "why" of this failure.
